Combined biochemical tests in the diagnosis of Cushing's
disease
RM Testa, N Albiger, G Occhi, F Sanguin, M Scanarini, S
Berlucchi, MP Gardiman, C Carollo, F Mantero and C
Scaroni. Cushing's syndrome can be difficult to diagnose
as it is difficult to differentiate between an
adenocorticotrophic hormone (ACTH)-secreting pituitary adenoma
and ectopic ACTH secretion by tumours. High-dose dexamethasone
suppression, CRH stimulation, desmopressin tests and bilateral
inferior petrosal sinus sampling (BIPSS) have been used for
differential diagnosis, with varying degrees of success. BIPSS
is considered to be reliable for establishing the origin of
ACTH secretion, but cost and relative risks limit its
availability. Testa and colleagues investigated the role of
these stimulation and suppression endocrine tests in the
diagnostic and therapeutic approach in patients with Cushing's
disease and negative pituitary MRI. Thirty one patients with a
diagnosis of Cushing's disease and who underwent
transsphenoidal pituitary surgery were analysed
retrospectively to determine whether accurate evaluation of
presurgical endocrine tests could reduce the number of
patients undergoing BIPSS. The authors' results indicate that
with accurate test evaluation, those patients with a negative
pituitary MRI could have BIPSS without affecting the success
of their post-surgical outcome. Recurrence rates in those not
receiving BIPSS were reported as being similar amongst
patients who recorded either positive or negative MRIs. EJE
